Keyboards and Cables

Hiya Guys and Girls.
Need some help with my keyboard wiring.
Following the schematic I get mixed results.
So could someone post pics on how they wired the keyboard.
I'm using the 4 momentary button type.
Yes, I know there's the schematic,but I think I'm missing something.
I think it maybe how they are grounded, just not sure.
Oh and pics of your isp and icsp programming cables too would be lovely.
